daily records behaivior/progress

Does anyone keep a daily log of the students progess/behavior? If yes, what infomation do you have on it? I'm new at this. Have a couple of problem students. Would like a standard form to log issues/progress.

In our para training they say it is good to keep track of what you are doing with kids and behavior that you encounter. I have never done it until this year. (Our principle likes to keep us para's in the bottom of the food chain-I feel that she thinks if we keep logs that we are in control of something and she doesn't want that! She has her own rules in our school) Our school didn't pass some state test so we have to show everything we are doing with the children. My general info consist of childs name, date and a brief decription of what I done with the child. Like: 10-19-Sandy-site words and HW (home work) help. This way if principle walks in she can see that I am working to better the children. This info is kept in a area within the childs classroom. In our para meeting several of the para's do keep a daily diary of what they do and they keep their diaries with them. This is a very good idea in case the "principle" forgot that you have complained about Joey being bad in lunch 3 weeks in a row. Hope this info helps a little.

In the classrooms I use a standard academic form. The other para's in the system just uses a little notebook to keep records.
Daily Progress Notes.
I work a school district in Oklahoma,we file for state monies to help with the cost of special needs children. Our state requires us to keep daily logs on our kids that we receive monies on. The para's turn daily logs into me every two weeks, and I make sure the notes are complete. I have a great form that I created with ms word, the para's love the form I have created for them, if you would like some help just email me at ljerri@yahoo.com. I don't mind helping anyone out.
